We work with individuals of all ages experiencing disabilities or challenges to their social and emotional well-being.
At Seachange Home Care Support Services, our vision and goal is to enable people living with psycho-social disabilities to achieve their goals and lead independent lives.
We are a proud registered NDIS Provider and strive to always provide our clients with the best possible services.
Who Is Seachange Home Care Support Services?
Seachange Home Care Support Services, established in 2018, has dedicated the past five years to offering vital assistance to individuals grappling with mental health challenges and disabilities.
Our primary mission involves delivering outreach support to those living with disabilities and coping with various mental health conditions, making a positive impact on their lives.
Our local support services are available on a Full-time, part-time or as-needed capacity.
We specialise in the South East Melbourne areas including Narre Waren, Clyde, Cranbourne, Berwick and the Mornington Peninsula and are wanting to expand to all of Melbourne

Meeting
We then set up a meeting between the client and the care team.
Support Worker
Using the results of the meeting we endeavour to find the best matched support worker to each individual client.
Schedule
We create a schedule that includes rostering shifts with our team to ensure ongoing support.
Communicate
With regular checkins from management we ensure we are kept up to date with every client including their own feedback.
